I am in-network with:
Aetna
Optum
United Healthcare
For these plans, I handle all billing and claim submissions directly through our client portal. Your responsibility will generally be your applicable copay, coinsurance, or deductible.
Out-of-Network Benefits & Self-Pay
Depending on your current health insurance provider, your plan may include out-of-network benefits that partially reimburse fees for my services. Please contact your provider to verify the reimbursement rates you would be eligible to receive.
Payment & Documentation: Clients are responsible for full session fees at the time of service. My practice provides monthly superbills solely as a courtesy for your personal records or self-filing.
No Practice Filing: I do not communicate with out-of-network insurance providers, track deductibles, or submit claims on your behalf.
Self-Filing Option: You may use Reimbursify, an independent app available for iOS and Android, to conveniently submit your superbill to your insurance company. All questions regarding benefits, reimbursement, or claim status should be directed to your insurance carrier.
Payment
I accept all major credit cards as forms of payment. Payments are due at the time services are rendered.
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours in advance to avoid being charged for the full rate of the session. Late cancellations or no-shows are billed at the full session rate, as insurance does not cover missed appointments.
Good Faith Estimate Notice
Effective 1/1/2022, a law went into effect called, The No Surprise Act, which requires health care practitioners to provide a Good Faith Estimate explaining your out-of-network health care costs. All self-pay clients who do not intend to seek insurance reimbursement or who do not have out-of-network benefits have the right to a Good Faith Estimate (GFE).
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under the law, health care providers need to give patients who do not have insurance or choose not to use their insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
